Our Support Coordination services empower you to get the most from your NDIS plan. We assist with understanding your budget, connecting with providers, reviewing service agreements, and ensuring you are getting value-aligned support. Our coordinators work with honesty and transparency, always placing your goals and choices at the centre of our work.

For participants with complex, high-risk, or crisis-driven needs, we offer Specialist Support Coordination (SSC). SSC involves intensive support planning and system navigation. We work with health services, housing, justice, child protection, or guardianship stakeholders to keep you safe and supported while maintaining your choice and control. Our SSC team brings clinical understanding, trauma-informed approaches, and crisis management capability.

This support can be used for:

• Navigating housing and homelessness services

• Coordinating therapy, medical, and daily supports

• Goal setting and plan review preparation

• Crisis support during system breakdowns

• Navigating child protection or justice involvement
