What is it?

What is support coordination?

Support coordination helps you understand your NDIS plan, find and connect with providers, and get the most out of your funding. It's funded through Coordination of Supports in the Capacity Building part of your NDIS plan. What a support coordinator does (ndis.gov.au).

We keep in touch, follow up and make sure nothing falls through the cracks. If your situation is complex, our Specialist Support Coordinators can work more intensively with you. We support people across the Fleurieu and the Adelaide Hills.

Who will support you?

You'll have a dedicated coordinator who takes the time to understand your goals, explains your funding and connects you with the right services quickly.

For complex or high-risk situations, our Specialist Support Coordinators work intensively with you, your family and your providers to keep supports safe and stable.

What does a support coordinator do?

They help you understand your plan, find providers, set up service agreements, solve problems and get ready for your plan review.

What is Specialist Support Coordination?

A higher level of support coordination for people with complex situations, like a housing crisis, hospital discharge or involvement with the justice system.

What is Psychosocial Recovery Coaching?

Support for people with a psychosocial disability to build skills, plan their recovery and connect with services. It's funded separately in your NDIS plan.

Can I change support coordinators?

Yes. It's your choice who coordinates your supports. We can help make the switch simple.
